Registering With Us

 

We welcome requests for registration from patients living in or moving to our practice area in Stockport.

You can register by either:

completing the form online or visiting us at the practice and collecting forms from reception.

Patients over 15 years old will be required to have a new patient medical with our health care assistant in order to be fully registered with the practice. The health care assistant will take your medical history, medication details, blood pressure, height and weight and refer you to the doctor or nurse if necessary. Medication will not be issued to patients until they have seen a doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

 

SAFE Surgery

We are proud to be a ‘Safe Surgery’ for everyone in our community. All are welcome. Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.  In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services. If you wish to register, please speak to a member of staff who can help and support you with the process of registration. Ask reception for an interpreter if you find it difficult to communicate in English.

Temporary patients

If you are not registered with a GP you will be unable to see one unless you have a serious emergency. In most other cases, calling 111 is a better option.

If you are not registered with a GP but need treatment at a GP Surgery you will need to complete a temporary registration form.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Child Registrations

A child can not be permanently registered without at least one person with parental responsibility registered at the same time (unless the child is in care or some other reasonable exception).  Cases will be considered on an independent basis and, if relevant, decisions will be made in reference to the out of area guidance outlined below. For example, if there is shared parental responsibility whereby one individual lives outside the boundary, how does impact the child’s ability to receive care.

 

Out of Area

The catchment area for GP practices serves to protect our patients by ensuring that there is no geographical restriction to accessing care. It ensures that we can effectively provide all levels of care to our patients. Therefore, we would not usually accept patients living outside of this boundary.

Subject to the criteria below, cases will be considered on an independent basis for remaining registered with us:

  • Your new address must be within the Stockport borough, this means we can still create onward referrals for treatment at other Stockport services and are still within the Stockport district nurse catchment area for treatment, should this be needed.
  • We are unable to provide home visits for patients outside of our catchment area. If your situation changes and you require home visits in the future, you will need to register with a new GP closer to home who can provide these services.
  • By remaining registered with us, you agree that Cheadle Medical Practice reserves the right to review this policy and give 28-day notice to any out of area patients for them to find a new practice at any time in the future.
  • Your ability to receive care cannot be impeded by the distance you live from the practice, for example if a face to face appointment is clinically required, this cannot be changed to a telephone appointment based only on it being too far to travel.
  • If at any point in the future you develop any complex care needs Cheadle Medical Practice reserves the right to remove you from our list if we feel we cannot sustainably offer the quality of care required.
