Cheadle Medical Practice uses RapidHealth triage system.
The system automatically determines a safe timescale for your problem to be seen by one of our highly-trained team of NHS clinicians. This means you don’t have to wait anymore for additional questionnaires to be sent out, or for the Triage GP to process your case. Simply complete the information on RapidHealth’s form, and if an appointment is needed, it will usually be offered to you there and then.
Some queries cannot be dealt with instantly and need to be checked by the GP. They may send you a prescription for a minor issue without an appointment, or refer you to a local pharmacy for treatment. This helps us offer appointments to more patients throughout the day.

Patients are asked to contact us online where you can, whether the matter is urgent or routine.

All within your local Stockport NHS practice! This means our GPs are available to see patients with the most complex and urgent queries.
Don’t worry if you can’t use the online option. You will always be able to phone us on 0161 983 9090. Reception can go through the relevant questions with you on the phone or at reception. If reception is busy we may need to call you back to get extra details.
All the answers still go through the RapidHealth system at the practice so you do not gain or lose priority by contacting us this way.
Multiple Issues
If you have multiple medical issues you would like to discuss, it’s best for the GP if you fill in multiple online forms. This helps us allocate appropriate time for the clinician to deal with all of your health needs. At busy times we may not be able to deal with all of your medical issues in one appointment. If your appointments are on different days/times or with different clinicians, you are very welcome to phone us to see if they can be seen in a single, longer appointment.
In Summary
Step One: Contact the GP practice via our website or phone to let us know about your medical issue.
Step Two: Complete a short questionnaire which usually takes less than 5 minutes.
Step Three: You will either be offered an appropriate appointment there and then, or for some conditions your query will come through to our team to manually check.
Extended Hours
We offer some evening and weekend appointments. Please indicate on your online form, or to the receptionist, if this would help you.
Checking, Changing or Cancelling an Appointment
You can do this using the NHS app, or by phoning reception on 0161 983 9090.
Home Visits
If you feel a Home Visit is needed, you should request this as close to 8am as possible. This helps us to resolve as many requests as we can, as quickly as we can. To request one, either complete our online form or phone reception on 0161 983 9090.
Chaperones
If you would like a chaperone for your consultation, please advise reception at any time. You may also ask the doctor/nurse during the consultation.
